I hereby grant [Full Name of Photographer] and their legal representatives and assigns, the irrevocable and unrestricted right to use and publish photographs of me, or in which I may be included, for editorial, trade, advertising, or any other purpose and in any manner or medium. I also consent to the use of any printed or electronic matter in conjunction with the images.
I acknowledge that I have no interest or ownership in the images, and that all rights to the images belong to [Full Name of Photographer]. I waive any right to inspect or approve the finished product or any printed or electronic matter that may be used in conjunction with the images, or the use to which it may be applied.
I release, discharge, and agree to hold harmless [Full Name of Photographer], their legal representatives or assigns, and all persons acting under their permission or authority or those for whom they are acting, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of the photographs or in any subsequent processing.
I have read this release form and fully understand its contents. I am of legal age and competent to execute this release, or I have obtained the consent of my parent or guardian to execute this release.
